The cause of this was neglect by ADOT when they ripped off the asphalt layer of the freeway and didn&#8217;t clean it up properly. If you drove through this you can get the repairs paid for by the state of Arizona. Below is how I filed my claim.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"894\" height=\"398\" data-src=\"https:\/\/new.jeremywhittaker.com\/wp-content\/uploads\/2021\/12\/image.png\" alt=\"\" class=\"wp-image-1138 lazyload\" data-srcset=\"https:\/\/new.jeremywhittaker.com\/wp-content\/uploads\/2021\/12\/image.png 894w, https:\/\/new.jeremywhittaker.com\/wp-content\/uploads\/2021\/12\/image-300x134.png 300w, https:\/\/new.jeremywhittaker.com\/wp-content\/uploads\/2021\/12\/image-768x342.png 768w, https:\/\/new.jeremywhittaker.com\/wp-content\/uploads\/2021\/12\/image-500x223.png 500w\" data-sizes=\"(max-width: 894px) 100vw, 894px\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 894px; --smush-placeholder-aspect-ratio: 894\/398;\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">Here are the steps:<\/h2>\n\n\n\n<ul class=\"wp-block-list\"><li>Fill out this form &#8211; <a href=\"https:\/\/drive.google.com\/file\/d\/1eGIXTzAYYcbIrEElOoHjlYQXvl6PhOpk\/view?usp=sharing\" target=\"_blank\" rel=\"noreferrer noopener\">NOTICE OF CLAIM AGAINST THE STATE OF ARIZONA<\/a> <\/li><li>Get quotes to fix your vehicle and include them<\/li><li>Take a picture of the damage print it out and send it with the form<\/li><li>Take a picture of your registration and send it with the form<\/li><li>Take a picture of your license plate and send it with the form<\/li><li>Google tracks everywhere you go if this feature is turned on using something called <a href=\"https:\/\/support.google.com\/maps\/answer\/6258979?hl=en&amp;co=GENIE.Platform%3DAndroid\" target=\"_blank\" rel=\"noreferrer noopener\">Google Maps Timeline<\/a>. I would include this as well showing travel through the area on that specific day. I don&#8217;t know if this is necessary but it makes it a lot harder to refudiate your claim.
